Name
Address including city and state
E-mail address
Phone number
Birth date
GHIN number and in some cases your partner's GHIN number
Credit/Debit card information
The way we use your information:
We use the information you provide about yourself or someone else when placing an order only to complete the order. We do not share this information with outside parties except to the extent necessary to complete that order
When registering online your GHIN number must be seven digits. If you have a six-digit number you must add a zero in front of your GHIN number to make it seven-digits.
